Registration for Summer I.D.E.A.S. 2018 is now open. Follow the steps below.

STEP 1 Registration Form

Download and print the appropriate registration materials (for ages 3-8 or grades 4-12).

Note: If using a single form for more than one child, be sure to include name, entering grade, and birthday for each child on the front page of the registration form. Please clearly indicate the programs selected for each child by writing the child's name or initials on the inside pages.

STEP 2 Medical Forms (As Needed)

If your child needs to take medication during the school day or has a severe allergy requiring an action plan, Summer I.D.E.A.S. requires that parents provide us with information necessary to ensure the well-being of their child.

In order to dispense medication, state and school guidelines require us to have written permission on file: 

• parent and physician signatures are required for prescription medication;
• parent-only signature is required for over-the-counter medication.

Complete the appropriate forms below and include with your registration materials.

Severe Allergy Action Plan

• New information is required each year. We do not hold forms from previous years.
• For school-year USM students, new forms are required as the Summer Programs Office does not have access to the files held in the nurse's office.

STEP 3 Payment

Payment is accepted in the form of cash, check, or in-person credit card payment option is available.

Payment in full by May 10, 2018 for programs running from June 11 through July 27 will qualify for an early registration discount. Regular fees are charged after May 10. The early registration discount due date for programs which start after July 27 is July 6 unless otherwise noted. 

Students will not be allowed to attend summer programs without tuition paid-in-full. There is no pro rata adjustment of the tuition charges based on the number of days attended. 

Classes that do not meet the minimum enrollment fourteen days prior to the starting date will be canceled. Participants will be invited to select a replacement program or receive a full refund.

STEP 4 Mail or Drop Off Materials

Once available, completed registration materials (registration form/s and cash or check) can either be dropped off at, or mailed to, USM. The mailing address is included with the registration materials.
