STEP 1 Registration Form
Download and print the appropriate registration materials (for ages 3-8 or grades 4-12).
Note: If using a single form for more than one child, be sure to include name, entering grade, and birthday for each child on the front page of the registration form. Please clearly indicate the programs selected for each child by writing the child's name or initials on the inside pages.
STEP 3 Payment
Payment is accepted in the form of cash or a check.
Payment in full by May 10, 2017 for programs running from June 12 through July 28 will qualify for an early registration discount. Regular fees are charged after May 10. The early registration discount due date for programs which start after July 28 is July 7 unless otherwise noted.
Students will not be allowed to attend summer programs without tuition paid-in-full. There is no pro rata adjustment of the tuition charges based on the number of days attended.
Classes that do not meet the minimum enrollment fourteen days prior to the starting date will be canceled. Participants will be invited to select a replacement program or receive a full refund.
STEP 4 Mail or Drop Off Materials
Once available, completed registration materials (registration form/s and cash or check) can either be dropped off at, or mailed to, USM. The mailing address is included with the registration materials.
